Evaluations are crucial to success in the education world. I have taught in the regular classroom- grades 3 and 4, resources gifted classroom grades K-5, college students, and graduate students. I always no matter what the age have students give me feedback on three questions so I can grow as a professional...
1. What am I doing well that I should continue doing in my class? (What do I need to continue?)
2. What am I doing that I shouldn't be doing in my class? (What do I need to stop or improve?)
3. What am I not doing that I should be doing in my class? (What something new and innovative to add?)
And of course other comments or suggestions are always welcomed and apart of my evaluation.

I like for parents and students both to give me this feedback. After I review this feedback, I look for patterns in improvements, strengths, weaknesses, etc....  Sometimes from my evaluations I will use those to set my goals for the year for my evaluation with my Chief of Academics and use them to guide me on what Professional Learning I will take part in this next year.

With my online courses I have utilized Surveymonkey, Moodle Evaluation, Google Forms and this has made it so much easier and quicker to get results to analysis.

Here is an example of some evaluations that I have done from previous classes I have taught.

This is a program evaluation that is given to teaches a year after they complete the gifted endorsement...
